Rick Lawson, Professional Computer Services (NC), a Business Associate, reported to HHS on 2009-12-11 a breach classified as Theft affecting approximately 2,000 individuals. During an IT vendor transition, a workforce member of the outgoing BA entered the covered entity's computer system, changed passwords, disabled accounts, removed drive mappings, deleted the backup program, and deactivated antivirus software. PHI affected included names, addresses, dates of birth, SSNs, appointments, insurance information, and dental records.
