Unity Recovery Group, Inc. and affiliated entities (FL) reported to HHS on 2015-05-11 an Unauthorized Access/Disclosure affecting 1,000 individuals. Breached information located on Email, Network Server, and Other Portable Electronic Device. Per OCR investigation, Unity had shared patient information with other covered entities for substance abuse treatment continuation and erroneously believed this to be an impermissible disclosure. OCR determined no actual breach occurred and provided technical assistance. Unity and all affiliates permanently closed December 31, 2015.
