Reading Health System (PA) reported to HHS on 2014-04-29 a Loss affecting 1,845 individuals. Breached information located on Paper/Films. A medical practice moved and a vendor/patient stored three boxes of paper medical billing records in the vendor's crawl space from March 2012 until March 2014. PHI exposed included names, addresses, dates of birth, SSNs, insurance information, billing codes, and diagnoses. The CE determined no disclosures had occurred and offered affected individuals free credit monitoring.
