An employee fell victim to an email phishing scheme, compromising PHI including names, dates of birth, diagnoses and conditions, full face photos, and treatment information stored in email. In response, the BA implemented MFA for all email accounts, increased password complexity, and established secure file-share/portal procedures for PHI transmission.
