Hospice of the Piedmont (VA) reported to HHS on 2021-08-02 a Hacking/IT Incident affecting 10,682 individuals. Several employees were victims of an email phishing scheme exposing PHI located in Email systems. Compromised data included SSNs, driver's license numbers, passport numbers, financial and health insurance information, diagnoses, and other treatment information. The CE offered free credit monitoring, implemented additional safeguards, and retrained staff on identifying fraudulent emails.
