Boston Baskin Cancer Foundation (TN) reported to HHS on 2015-02-02 a Theft affecting 56,694 individuals. On December 2, 2014, an employee's laptop and unencrypted external hard drive were stolen. The drive contained ePHI including names, dates of birth, SSNs, addresses, phone numbers, medical record numbers, and dates of service. The CE notified affected individuals and the media, offered credit monitoring, deployed encryption controls, updated mobile device passcode policies, and retrained staff.
