Spectrum Health Lakeland (MI) reported to HHS on 2019-05-06 a Hacking/IT Incident affecting 1,100 individuals. An employee of the CE's vendor was the subject of an email phishing scheme that compromised PHI stored in email. PHI involved included names, dates of birth, Social Security numbers, diagnoses, and claims information. The CE notified HHS, affected individuals, and the media, posted substitute notice on its website, and implemented additional administrative and technical safeguards. Workforce members were retrained on email security precautions.
