JFK Medical Associates, PA (NJ) reported to HHS OCR on 2021-06-18 a ransomware attack affecting 1,304 individuals, with breached information located on a Network Server. PHI involved included names, addresses, dates of birth, Social Security and drivers' license numbers, state ID numbers, health insurance information, and diagnosis/treatment data. The CE notified HHS, affected individuals, and the media. In response, complimentary credit monitoring was offered and additional administrative, technical, and security safeguards were implemented.
