Detroit Department of Health and Wellness Promotion (Detroit, MI) reported to HHS OCR on 2009-12-15 a Theft affecting 646 individuals. A desktop and four laptop computers were stolen from the covered entity's locked facility. PHI involved included names, addresses, dates of birth, Social Security numbers, types of services received, and Medicare/Medicaid numbers. Post-breach, the entity installed new locks and cameras, physically secured computers, centralized ePHI storage, and conducted OCR-mandated workforce training.
