On February 11, 2013, a laptop was stolen from the William Jennings Bryan Dorn VAMC's Pulmonary Testing Unit in Columbia, SC. The laptop contained PHI of approximately 7,405 individuals, including names, dates of birth, and clinical information. The CE notified HHS, media, and affected individuals, offered credit monitoring, and filed reports with VA police and the VA OIG. Physical safeguards for laptops on medical devices were improved and secure-storage procedures implemented. OCR obtained assurances that all corrective actions were completed.
