On March 14, 2010, a computer hard drive was lost or stolen from the Alzheimer's Registry Program Department. The hard drive contained ePHI including patient names, addresses, dates of birth, Social Security numbers, diagnoses, and treatment information. Breached information was located on an Other Portable Electronic Device. No business associate was involved. The CE conducted risk analyses, retrained staff, notified affected individuals, and provided one year of credit protection.
