Resource Anesthesiology Association of California reported to HHS on 2021-09-05 a Theft affecting 2,700 individuals. An employee's laptop was stolen from a vehicle. The PHI involved included names, addresses, dates of birth, diagnoses, and health insurance and other treatment information. Breached information was located on a Laptop and Paper/Films. The CE notified HHS, affected individuals, and the media, provided substitute notice, and offered complimentary credit and identity theft monitoring. Additional safeguards were implemented and workforce retrained. OCR provided technical assistance.
