Intervention Services, Inc. (FL) reported to HHS OCR on 2013-02-07 a theft affecting 1,200 individuals. A laptop was stolen from a workforce member's vehicle. The ePHI on the laptop included patient names, dates of birth, Medicaid numbers, and names of patients' funding sources. The CE filed a police report, notified affected individuals and the media, improved physical security, sanctioned the workforce member, and retrained staff. OCR obtained assurances of corrective action.
