Flowers Hospital (Dothan, AL) reported to HHS on 2014-04-25 a Theft breach affecting 629 individuals. On February 27, 2014, law enforcement notified the hospital that paper facesheets (clinical and demographic information) were found in a hospital employee's possession upon arrest. An internal investigation indicated the employee may have accessed or shared the records of up to 1,208 individuals. Breached information was located on paper/films. The CE notified HHS, affected individuals, and the media, and implemented corrective access-control and storage procedures. OCR obtained assurances of remediation.
