Austin Manual Therapy Associates (TX) reported to HHS OCR on 2017-12-06 a Hacking/IT Incident affecting 1,750 individuals. From October 3–9, 2017, an unauthorized individual accessed a CE laptop and a shared file folder. PHI exposed included names, addresses, dates of birth, Social Security numbers, diagnoses, treatment information, and claims data. Breached info located on Laptop and Network Server. CE remediated by network isolation, firewall updates, MFA rollout, and audit controls. OCR obtained assurances of voluntary corrective action.
