Retina Associates of Cleveland, Inc.

Retinal Consultants Medical Group, Inc. (dba Retinal Consultants Medical Group) reported the theft of a laptop containing unsecured Protected Health Information (PHI) on June 7, 2013. The laptop, part of a diagnostic imaging machine, was stolen after June 5, 2013. The PHI included names, dates of birth, gender, race, and OCT images. No SSN, Driver's License, or address was on the device. The incident was reported to local police, and the organization enhanced physical and interior security measures.
