Meigs County EMS (OH) reported to HHS OCR on 2016-12-05 a ransomware attack detected on October 6, 2016 affecting 817 individuals. Hackers attacked the computer server (Desktop Computer, Email, Network Server) and may have acquired patients' PHI including demographic, financial, and clinical information. The CE notified HHS, affected individuals, and the media, and implemented physical, administrative, and technical safeguards, including an audit removing unnecessary/stale network accounts. OCR obtained corrective action assurances.
