Jewish Social Services Agency (MD) reported to HHS on 2019-12-12 a Hacking/IT Incident (email phishing attack) affecting 3,145 individuals. An employee was victimized via phishing, exposing ePHI stored in Email systems, including names, dates of birth, addresses, health insurance information, Social Security numbers, and treatment information. The CE notified HHS, affected individuals, and the media, and provided credit monitoring and identity protection services. Additional technical safeguards were implemented post-breach.
