Paul C. Gering, Jr., M.D. reported to HHS on 2017-06-29 a Theft affecting approximately 2,000 individuals. On June 2, 2017, burglars broke into the office and stole a desktop computer server. PHI exposed included patients' addresses, dates of birth, driver's licenses, names, Social Security numbers, clinical diagnoses/conditions, lab results, medications, and other treatment information. The CE notified law enforcement, HHS, affected individuals, and the media. Physical safeguards were improved by relocating the server to the basement and installing security cameras. OCR obtained assurances of corrective actions.
