On November 4, 2014, an employee’s password-protected laptop and external hard drive were stolen from a locked vehicle. The devices contained the protected health information of 2,855 individuals, including names, addresses, dates of birth, Social Security numbers, financial data, and clinical information. The covered entity, Murali Menon and Physicians Skin and Weight Centers, notified HHS on December 30, 2014. In response, the entity discontinued the use of external hard drives, encrypted all laptops, revised security policies, and retrained staff. Affected individuals were offered one year of free credit monitoring.
