Northern Iowa Therapy PC (a Business Associate) reported to HHS OCR on 2023-10-29 a Hacking/IT Incident affecting 5,112 individuals. The cyberattack compromised PHI stored on a Network Server, including names, addresses, dates of birth, Social Security numbers, phone numbers, email addresses, diagnoses and conditions, and health insurance/treatment information. The BA notified HHS, affected individuals, and the media, and implemented additional administrative and technical safeguards. OCR provided technical assistance on privacy compliance.
