John J. Pershing VA Medical Center (MO) reported to HHS on 2013-04-11 a Theft affecting 589 individuals. Breached information located on Paper/Films. Business associate Stress Laboratory placed a box of unsecured PHI (names, Social Security numbers, diagnoses, and age) in an equipment storage room. The BA employee involved separated from employment in 2012; the BA was reorganized and incorporated into the CE. OCR opened an investigation and obtained assurances of corrective actions, including new PHI-handling policies and employee retraining.
