AvMed, Inc. (FL Health Plan) reported to HHS OCR on 2010-06-03 a Theft affecting 1,220,000 individuals. Two laptops with questionable encryption (each holding ePHI of ~350,000 individuals) were stolen from CE premises. ePHI included demographics, diagnoses, medications, lab results, and other treatment data. CE reported theft to law enforcement, notified all affected individuals, HHS, and media, and implemented new HIPAA Security Rule policies. Breached information located on Laptop.
