On February 29, 2016, Atique Orthodontics reported that unauthorized access to its web server compromised files containing the PHI and PII of approximately 1,506 individuals, including names, DOBs, SSNs, and credit card numbers. The entity disconnected the compromised computer, disabled remote access, upgraded security software, encrypted ePHI, and implemented new policies and training. HHS notification was provided, and identity theft protection was offered.
