Clinical & Forensic Consultation, LLC d/b/a Thriveworks Bristol, Johnson City, and Knoxville (TN) reported to HHS on 2020-10-07 an Unauthorized Access/Disclosure affecting 1,360 individuals. An employee impermissibly disclosed the email addresses of 1,360 individuals via email. The CE sanctioned the responsible staff member and retrained all staff on ePHI safeguarding procedures.
