The Kent Center (RI) reported to HHS on 2010-09-10 a Theft affecting 1,361 individuals. A briefcase containing paper documents with PHI was stolen from an employee's car. PHI included clients' names, dates of birth, and limited clinical information for some. The CE sanctioned the employee, revised its confidentiality and breach notification policies, and retrained staff. OCR investigation led to further policy updates.
