Fayetteville VA Medical Center (Fayetteville VA Medical Clinic Optical Shop) reported to HHS on 2013-06-14 a Theft/improper disposal incident affecting 1,093 individuals. From January to April 2013, consultation reports containing patients' names, SSNs, birthdates, addresses, and phone numbers were placed in a recycling bin rather than the shred bin. Breached information was on Paper/Films. The CE notified affected individuals, offered credit monitoring, shredded documents, retrained staff, and sanctioned the responsible employee. OCR obtained assurances of completed corrective actions.
