Liberty Resources, Inc. (PA) reported to HHS OCR on 2012-08-17 a Theft affecting 3,183 individuals. An employee's personal laptop containing unencrypted ePHI was stolen from his vehicle. Breached data included consumer names, identification numbers, diagnosis codes, procedure codes, service/authorization details, funding sources, and provider information. The CE notified all affected individuals, the media, and HHS, offered fraud-alert assistance, and implemented new policies including full-disk encryption of agency laptops. OCR obtained corrective-action assurances.
