Community Catalysts of California

Community Catalysts of California reported the theft of a thumb drive from an employee's car on September 8, 2015. The device contained personal information including names, addresses, diagnoses, dates of birth, and Social Security numbers for some clients. The organization notified law enforcement, investigated the contents of the drive, and implemented remediation measures including staff retraining and mandatory device encryption. Identity protection services were offered to affected individuals.
