Austin Perio Health (TX) reported to HHS on 2021-11-17 a Hacking/IT Incident (email phishing attack) affecting 4,339 individuals. An employee was the victim of a phishing attack that exposed PHI including names, dates of birth, Social Security numbers, drivers' license numbers, health insurance information, and treatment information.
