A trustee of Capron Rescue Squad District (IL) removed a laptop containing unencrypted ePHI of 815 individuals, mistakenly believing it was no longer in use, and gave it to his adult grandson. The ePHI included full names, SSNs, dates of birth, home addresses, and medical histories. The laptop was recovered; written assurances were obtained from those involved. The CE notified HHS, media, and affected individuals, and subsequently encrypted ePHI on all computers. OCR confirmed corrective actions were completed. Breached information located on Laptop.
