Blain Jacobson DMD, CAGS reported a data breach involving unauthorized transfer of patient records during a provider transition with A Plus Dental Group. A fraudulently prepared transfer form was used to move Invisalign patient lists without authorization. Affected data includes patient names, photographs, digital dental models, radiographs, and treatment plans. The incident was contained after Invisalign returned patients to the appropriate account.
