Alexian Brothers Medical Center (IL) reported to HHS on 2015-05-19 an Unauthorized Access/Disclosure affecting 632 individuals. On April 13, 2015, several files containing ePHI were discovered on publicly accessible computers in the medical library. Exposed data included patient names, medical record numbers, and medication/clinical information. Breached information was located on Desktop Computers. The CE remediated by securing public computers, rendering folders read-only, implementing access tracking, and retraining staff. OCR obtained documented assurances of corrective action.
