Best Health Physical Therapy, LLC (BHPT) reported to HHS on 2016-11-10 an Unauthorized Access/Disclosure affecting 1,100 individuals. An internet security blogger searching for vulnerable data impermissibly accessed the computer systems of Rehab Billing Solutions, BHPT's business associate (BA). PHI exposed included names, addresses, dates of birth, insurance information, driver's license information, and health information. BHPT notified HHS, affected individuals, and the media, and offered two years of free credit monitoring. BHPT subsequently terminated its relationship with the BA, which then ceased operations.
