NOVA Chiropractic & Rehabilitation Center (VA) reported to HHS on 2014-03-27 a loss incident affecting 5,534 individuals. The CE misplaced a mobile device (other portable electronic device) within its office containing ePHI including names, dates of birth, and addresses. No evidence of misuse outside the office was found. OCR investigation led to thumb drive encryption, new security alarm installation, updated PHI removal policy, and execution of BAAs for email and cloud providers.
