Southern Perioperative Services, P.C. (AL) reported to HHS on 2010-12-30 a Theft affecting 2,000 individuals. A bag containing a CD-ROM was stolen from a physician's vehicle. The CD-ROM held names, dates of birth, SSNs, medical histories, and treatment information of approximately 2,046 individuals. The CE filed a police report, notified affected individuals and media, sanctioned/retrained the physician, and implemented organization-wide Privacy and Security Rule improvements. OCR's investigation led to substitute notification in a local paper.
