PHI involved included names, addresses, telephone numbers, diagnoses, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and media, and implemented new technical safeguards and revised policies and procedures. OCR provided technical assistance regarding the HIPAA Breach Notification Rule.
