Connally Memorial Medical Center reported that an employee was the target of an email phishing incident that affected the protected health information (PHI) of 1,228 individuals. The PHI involved included names, dates of birth, Social Security and drivers’ license numbers, addresses, medications, diagnoses/conditions, claims and financial information, and other treatment information. The company notified HHS and affected individuals, offered free credit monitoring, implemented additional safeguards, and retrained staff.
