Computer backup tapes containing EPHI (electronic medical records, names, addresses, phone numbers, dates of birth, insurance information, SSNs, and medical record information) were stolen from the home of the practice manager on December 11, 2009. Breached information was located on Electronic Medical Record, Other, and Other Portable Electronic Device media. Corrective actions included encryption, revised transport/storage policies, and employee retraining.
