Golden Care Pharmacy LLC (NJ) reported to HHS OCR on 2021-06-14 an Unauthorized Access/Disclosure affecting 5,530 individuals. An employee impermissibly accessed and altered PHI stored on Desktop Computers, Electronic Medical Records, and Network Servers. PHI involved included names, addresses, dates of birth, driver's license numbers, medications, and claims information. The DOJ subsequently charged the owner with conspiracy to commit health care fraud and four counts of health care fraud; OCR closed its investigation accordingly.
