Community Action Partnership of Natrona County (WY) reported a Theft breach to HHS on 2011-04-20 affecting approximately 15,000 individuals. The breached information was located on a Desktop Computer. The CE initially reported that a virus had infected the computer and exported data; however, OCR's subsequent investigation determined that no PHI was actually exported or breached. As a result of OCR's compliance review, the CE improved virus/malware safeguards, conducted a risk analysis, drafted a risk management plan, and revised its HIPAA policies and procedures.
