Mercy Health Physician Partners

This breach affected 2,487 individuals and the PHI involved included names, addresses, financial information, and clinical data. The CE notified HHS, affected individuals, the media, and established a toll free number for questions or concerns. In its mitigation efforts, the CE sanctioned the responsible employee, implemented additional administrative safeguards, and retrained its staff on the privacy and security of PHI. OCR obtained assurances that the CE implemented the corrective actions noted.
