LaBorde Therapy Center LLC (LA) reported to HHS on 2019-10-16 a Hacking/IT Incident affecting 7,000 individuals. An employee was the victim of an email phishing attack that compromised ePHI including names and email addresses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response, additional technical safeguards were implemented and staff was retrained on recognizing fraudulent emails. OCR obtained assurances that corrective actions were completed.
