St. Martin Parish School Based Health Centers (LA) reported to HHS OCR on 2015-06-15 a theft affecting 3,000 individuals. A break-in occurred on April 30, 2016 at the Cecilia School Based Health Center clinic, during which four desktop computers, one laptop, a wireless router, and several printers were stolen. PHI exposed included names, addresses, dates of birth, Social Security numbers, diagnoses, and procedure codes. Breached information located on Desktop Computer, Electronic Medical Record, and Laptop. OCR obtained assurances that corrective actions were implemented.
