Jesle Kuizon, a Business Associate, reported to HHS on July 18, 2013, a breach affecting 800 individuals that occurred between October and November 2011. Employees of San Jose Medical Supply, Inc. (SJMS) impermissibly disclosed patient information from a desktop computer and network server. The disclosed data included names, addresses, medical conditions, diagnoses, and license numbers. In response, SJMS initiated a forensic investigation, notified the California Attorney General and affected individuals, and enhanced its security measures.
