Samaritan Center of Puget Sound (WA) reported to HHS OCR on 2021-09-15 a Theft affecting 20,866 individuals. A desktop computer and network server containing PHI were stolen from the facility during a burglary. PHI exposed included names, addresses, Social Security numbers, health insurance information, financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Remediation included strengthened physical and additional technical safeguards.
