Alberto Gerardo Vazquez Rivera (a Business Associate/AFLAC associate) reported to HHS on 2013-06-28 a Theft affecting 679 individuals. An encrypted laptop was stolen from an AFLAC associate's vehicle in Puerto Rico. The laptop contained PHI including patient names, addresses, birthdates, SSNs, claims information, and diagnoses. A police report was filed, all affected individuals were notified, and the responsible workforce member was sanctioned. OCR noted the incident does not constitute a reportable breach as the laptop was sufficiently encrypted.
