Affiliates of Nexion Health, Inc. (MD) reported to HHS on 2025-12-12 an Unauthorized Access/Disclosure affecting 1,027 individuals. Breached information was located in Email. An employee emailed the PHI of 1,027 individuals to the wrong recipients. PHI included names, addresses, dates of birth, claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In response, the CE retrained workforce members and implemented new technical safeguards.
