Hope Healthcare Service (FL) reported to HHS on 2021-02-11 a Hacking/IT Incident affecting 1,152 individuals. The covered entity's business associate experienced a ransomware attack that compromised PHI stored on a Network Server. PHI exposed included names, dates of birth, addresses, Social Security numbers, diagnoses, and other treatment information. The CE and BA notified affected individuals and media. OCR provided technical assistance regarding the HIPAA Privacy Rule.
