MHMR of Tarrant County (TX) reported to HHS OCR on 2020-01-31 a Hacking/IT Incident (email phishing) affecting 6,524 individuals. Two employees fell victim to a phishing scheme, compromising ePHI stored in email. Exposed data included names, addresses, birthdates, Social Security numbers, treatment information, and driver's license numbers. The CE notified affected individuals and the media, implemented new administrative and technical safeguards, and retrained staff on identifying fraudulent emails. OCR obtained assurances of corrective action.
